Helpful tips for filling out Adobe® Acrobat® forms using Adobe® Acrobat® Reader.
    Important Note: Completed forms CANNOT be saved in an electronic format for your future reference using Adobe® Acrobat® Reader. Saving the file will only save an additional blank copy of the form to your local computer. Please keep a printed copy of the completed form for your permanent records.

  • All of the forms accessed from this Website can be filled out prior to printing using Adobe® Acrobat® Reader 3.0.
  • Forms accessed from this Website may contain any combination of the following data entry fields: text fields, radio buttons, and check boxes.
  • Once a form has loaded into Adobe® Acrobat® Reader 3.0., select the "hand" tool from Reader's menu bar to enable data entry. The hand will change into another symbol as you move it across a data entry field. For instance, the hand will change into a text bar over a text field or a pointer over a radio button or check box.
  • To enter data, simply click inside a text field and a curser will appear, or click on a radio button to select. Check boxes may not be as obvious so we have tried indicate where items should be checked. A check box may be selected in the same manner as a radio button. Unlike radio buttons, check boxes may be deselected by clicking on the checkmark.
  • Once a text field has been selected, you may use the tab key to easily jump to the next text field.
  • Please Be Aware! The number of characters that may be entered into most of the text fields is limited. In most cases, these limits were dictated by the physical space limitations on the form. In the event that a text field will accept more characters than the space provided, some characters may scroll out of view if you enter more data than space allows. Only the visible text will print so review data carefully before printing. top
